This compulsory complementary health insurance, specific to the departments of Haut-Rhin , Bas-Rhin and Moselle, and inherited from the very generous German social legislation of the beginning of the 20th century, considers that “the favorable context” of its financial situation allows it to envisage such a drop, which had not taken place since 2012.

For the 1.6 million insured, this will correspond to a gain of 64 million euros on their salaries (i.e. around €3 per month for an employee with the minimum wage) that the managers of the local scheme wish to see reinjected into the economy. local.

The effort “remains bearable”

The scheme has also decided to extend the care of adult children until the eve of their 24th birthday, and no longer 20 years old – which will concern 33,500 young people at a cost of 4 million euros. It will also quadruple its public health prevention actions (+2.3 million).

All of these decisions should lead to a deficit of around 20 million euros in 2022, but the effort is judged “tolerable” given the 385 million euros in plan reserves (ie 75% of annual benefits). The 2020 financial year had a surplus of 40.7 million euros.

The “local health insurance scheme for the departments of Haut-Rhin, Bas-Rhin and Moselle” is a legacy of the social security created when these departments were under the sovereignty of the German Empire. This had been maintained when Alsace-Moselle returned to France in 1918, in particular the Reichsversicherungsordnung (“Imperial insurance regulations”) of 1911 imposing compulsory complementary mutual insurance.

In 1945, the creation in France of Social Security, which imposed a co-payment of 20% on health care expenses, had generated protests in Alsace-Moselle: the local supplementary scheme had then been “provisionally” maintained until the new general scheme can offer equivalent benefits. Which has never been the case…

Two laws of 1990 and 1994 finally perpetuated the system, managed by a board of directors of 25 members (including 23 representatives of employee unions). It now benefits any employee carrying out his activity in Alsace-Moselle or itinerant workers attached to an establishment which has its head office there.

This represents 2.1 million people (54% of employees, 25% of entitled persons and 21% of retirees and invalids), i.e. 67% of the insured persons of the general scheme of the three departments (the self-employed, attached to the general scheme are not not affected).

Cheaper mutuals

In addition to additional coverage of medical costs (90% of doctors’ fees, compared to 70% in “inland France” or 90% of drugs reimbursed elsewhere at 65%), the scheme also contributes to the prevention of three diseases particularly affecting Alsace-Moselle: cancers, chronic bronchitis and cardiovascular diseases. Thanks to the local scheme, complementary mutuals are also less expensive in Alsace and Moselle than in the rest of France.
